Life Care at Home brings trusted, personalized care to older adults living at Montgomery Place and throughout the greater Chicago community. Whether you live in one of our residences or your own home, our mission remains the same: to help you live safely, independently, and with dignity.

As part of Montgomery Place, our services reflect the same thoughtful, person-centered approach. We meet you where you are—and support you in staying there, comfortably.

A Personalized Match—More Than Just a Caregiver

What sets Life Care at Home apart is our focus on meaningful partnerships. Your care partner will be in your home regularly, and that relationship matters. That’s why we carefully match each client with a caregiver whose personality, communication style, and approach align with your own.

Our pairing process begins with a comprehensive in-home assessment to learn about your daily routines, values, interests, and preferences. From there, we create a customized plan and match you with a team member who will not only meet your needs—but truly enjoy your fellowship.

It’s more than care—it’s a relationship built on trust.

Post-Hospital & Respite Care

Short-term support during transitions or caregiver breaks.

Returning home after a hospital stay—or caring full-time for a loved one—can be overwhelming. Life Care at Home steps in with flexible, short-term care to bridge the gap or offer needed relief.

We assist with:

  • Recovery from surgery or illness

  • Medication and wound care monitoring

  • Daily support during rehabilitation

  • Temporary care when caregivers need time away

Our goal is a safe, supported recovery—and peace of mind for everyone involved.

How Is Life Care at Home Different from Home Health?

Life Care at Home is a private home care agency. That means we offer long- or short-term support based on your personal needs and preferences—not just what insurance allows.

By contrast, home health is typically ordered by a physician to provide medical or therapy services for a short time, and is often covered by Medicare or private insurance. If you’re looking for a trusted care partner on your terms, we’re here to help.

What Is the Cost?

Because every care plan is unique, costs are determined following your personalized assessment. We work with you to develop a plan that fits your needs, goals, and budget.
